Open Contacts, pick the person, tap Edit, then tap Ringtone or Text Tone and turn on Emergency Bypass. Repeat on the other one if you want both.
Apple also lets calls from the emergency contacts in your Medical ID come through when notifications are silenced, which is worth setting up alongside this if you have not already. Focus settings offer a related but separate control, Allow Repeated Calls, which lets a second call from the same number within three minutes ring through.

- Tap Edit.
- Tap Ringtone, turn on Emergency Bypass, then tap Done.
- Repeat under Text Tone if you want their messages to break through too.
